These forms are intended for healthcare professionals who wish to refer their smoking patients to the I QUIT NOW services. The specialists of the I QUIT NOW line will receive the completed forms and will offer patients the help of the I QUIT NOW line consisting of:

• online assistance offered by the I QUIT NOW website;
• telephone assistance offered by the I QUIT NOW line;
• in-person assistance offered at smoking cessation centers in their area;
• text assistance offered by SMAT.
